If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered / entitled to a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. 

You can collect a registration form and health questionnaire directly from the surgery or you can use the links below to download the forms and email them directly to Tay.Stanley@nhs.scot  (please do not use the link on 'Contact Details' to send in your registration forms).

You only have to complete the New Patient Registration Form (the Health Questionnaire allows us to update more information until we receive your full medical records - this is optional).
